Add book size options for (annoying) American standards

This is needed as many American P-O-D providers don't support the sizes that Booki currently exports to. Example:

Ideally, we should provide a custom size input too.

Objavi can take arbitary sizes. It is just that Booki doesn't expose those options.

There are some changes pending to booki and objavi that will allow you to use the objavi interface to make booki books (at present objavi only lists old FM books), which will make this change less urgent for booki.

For now:

1. Use the form at  http://objavi.flossmanuals.net/

2. choose page size "custom"

3. put in any numbers you want less than 1000 x 1414.

It is true that the page size list is arbitrary and strange. If you have new suggestions, I will consider adding them too.

Also, i realised custom Page size option is already in booki interface. CSS auto hide option of width/height inputbox is missing when you select "custom" option, but it works if you enter values inside. Will fix this hide/show part later. Probably also put some other options for objavi.

Yes, I've started using that, it works well :)
I would recommend adding the following sizes:
5.5"x8.5"
5"x8"
6"x9"
7"x10"

Maybe these should actually be divided to American industry standards vs. ISO

Replying to mushon:

6"x9"

This was already there, but called "USTRADE". Now "USTRADE6x9".

5.5"x8.5"
5"x8"
7"x10"

Added these ("US5x8", etc).

Most of these are there also (e.g., 6.14 x 9.21 is "ROYAL"). These are presumably the names they've got on Lulu.com.
