Put new chapters into TOC

Currently new chapters get pout into the 'hold' area. However I have noticed many books exist with chapters in hold but none in the TOC. Thats because I think people dont realise the chapters need to be dragged into the TOC area and hence they might think booki 'doesnt work'.

I have tried to remedy this by putting a text on the edit template to encourage people to drag content up. However I dont think this is the solution, I think a new chapter should go to the top of the TOC - position 1. Afterall, new chapters are presumably created to go into the book...so it doesnt make much sense ot have them auto go into the hold area. Sections, incidently, go immediately into the TOC when created.
